Medical Records Request

We maintain patient medical records in accordance with statutory requirements. If you wish to request records from Eastside Urgent Care, please follow the instructions below.

Patients

Patients will need to fill out a Medical Records Release form (available at the link below or at our clinic). The form should then be emailed to our medical records department (contact information provided below). After we receive your form, it typically takes us two business days to process your request. After this period, please visit our location with your printed form and photo ID to retrieve your records.

Physicians & Medical Facilities

Physicians or medical facilities can send a request through email, fax, or mail with a letterhead. The request typically takes 24 hours to process.

Attorney & Others

Attorneys or parties not listed above can send a request through email, fax, or mail with a letterhead. The request requires a properly executed medical release and prepayment in full. In most instances, records will be ready within two weeks of receiving your prepayment.
